SAMBA, in it's latest version, claims to provide the UNIX list to the
Windows machines when acting as a PDC, and to use the kernel ACL
patches if installed to provide WinNT-like ACLs.

In order to have UNIX-like file permissions, you'd make SAMBA a PDC,
then make the Windows machines 'Login to network' on boot (by selecting
'Login into Microsoft Network' instead of 'Login into Windows' in
Control Panel - Networks). After succesfully loging in (your password
will be authenticated by SAMBA), you could use the exported directories
with the permissions of the UNIX user you logged-in as.

When I tried it two years ago, it couldn't authenticate against the
master password file (/etc/shadow) so I had to add users explicitly to
a /etc/smbpasswd file (I think they had a 'smbpasswd' utility for
that). Things probably changed since then though.

SAMBA takes care of that, making files case-insensitive.

That's totally transparent to the ext2 filesystem.
