Hi,
I do not like any Lin/Winmodems at all, I like to be free
to upgrade my kernel at will.
The only exception to this rule is the IBM laptops, I don't
know of any laptop with hardware modem, the Lucent hack seem
to work pretty well up to 2.4.6, I did not try it with 2.4.7.
I found PCI hardware modems in Israel:
IBM ( You need to be in close relationship with your
IBM dealer).
There is also Zoom, Controller based PCI (abnet is selling Zoom).
Both of the above are not in stock, but if you have a few weeks.....
No cheap bargains here, Hardware PCI costs ~80$ Zoom and over ~120$ IBM.
External: You can get the serial ones, mainly 3Com.
On the USB front there is Zoom, Dlink and 3Com.

There must be more, but I also found non working USB modems (Intel),
So be careful here too.
Bye
Gal
