I have seen this problem with Slackware 2.2/March 1995 CDROMs. Probably
a bad "bash", try to replace it with a newer (or older) version. Be
careful about it, save the original under some other name (mv /bin/bash
/bin/bash.bad and mv bash.new /bin/bash) and don't logout from root or
reboot until you are sure that the new one is working.

This has to do with the color map. The default - 8 bpp is not enough for
xv, netscape, etc. Depending on your video card and whether it has enough
RAM(>= 2MB) try to run the X server with "-bpp 16" (16 planes).
